“Herniated lumbar disc at L5-S1 with bilateral nerve root dysfunction; bulging lumbar discs at L3-4 and L4-5 with desiccative changes at all three levels requiring a bilateral decompressive laminotomy/facetectomy at L3-4 and L4-5; cervical spondylosis with spinal stenosis and foraminal narrowing.”
